Print out a current Lifelong Brochure (pdf) for yourself or a client.
Generic Referral Form (pdf): Use this form to give us information
you think is important about the person you are referring to us.
Any and all information about clients referred for evaluation is
welcomed! The better you are able to articulate the reason you are
referring a person to us, the better we will be able to meet your needs
and the needs of your client.
Release of Information (pdf): Please use this HIPAA compliant form
to request client records. Blank forms are unacceptable, so please
remember to have your client sign the form and fill it out completely,
then fax or mail it to us.
We charge the same rates to all clients although some contracts with
larger agencies may reflect the previous year's fee structure.
