Fees and Forms of Payment
Services
and Fees PDF Payment is due at the time of service unless
other arrangements have been made in advance. We accept cash,
personal checks, Visa and MasterCard and will work with you to submit
insurance claims as an out of network provider when this is appropriate.
Please ask about which insurances we currently accept. For
learning disability and AD/HD evaluations only, college students are
eligible for a 25% discount on the current rate (see
fee sheet). This discount does not
apply for students sponsored by an agency such as the Division of
Vocational Rehabilitation or agencies with whom we have contractual
agreements
